Referrals Welcome

Welcoming Adults with Developmental Disabilities

Cornerstone House accepts referrals for adults with intellectual and/or developmental disabilities who require residential group home supports. We work with Virginia Medicaid Waiver programs and coordinate with support coordinators, Community Services Boards (CSBs), and families.

Three Simple Steps to Admission

We make the referral process as straightforward as possible for support coordinators and families.

Submit a Referral

Complete our referral form or call (804) 555-0199. Provide basic information about the individual and their current support needs. We respond within one business day.

Assessment & Visit

We review documentation, conduct an initial assessment, and invite the individual for a welcome visit. This ensures our home is the right fit for their needs and preferences.

Transition & Move-In

We collaborate with support coordinators, families, and current providers to create a personalised transition plan — minimising disruption and ensuring a warm, welcoming move-in.

What We Typically Request

The following documents help us understand the individual's needs and plan their support effectively. We'll guide you through the process.

  • Current Individualized Support Plan (ISP)
  • Level of Functioning (LOF) assessment
  • Psychological evaluation (if available)
  • Medical history and current medication list
  • Behavioral support plan (if applicable)
  • Insurance / Medicaid Waiver approval documentation
  • Emergency contact and guardian information
